Media Consent Form

The Calgary Youth Curling Association (the “CYCA”) is dedicated to supporting and enhancing the sport of curling among Calgary’s youth. One component of the CYCA’s programs involves marketing and promoting youth curling to the local community through means including, but not limited to, websites, social media, brochures, newsletters, and radio and television features (collectively, the “CYCA Promotional Activities”).

The CYCA Promotional Activities involve the collection, use, and disclosure of video, sound, or photographic recordings of youth curlers in respect of current and future CYCA Promotional Activities. As such, the CYCA is hereby seeking the voluntary consent of youth curlers (and of their parents/guardians) for such collection, use, and disclosure to occur.

By signing below, the youth curler and his/her parent or guardian agree as follows:

1. The youth curler and his/her parent or guardian voluntarily consent to the CYCA’s collection, use, and disclosure of video, sound, or photographic recordings of the youth curler for purposes related to current and future CYCA Promotional Activities;

2. The youth curler and his/her parent or guardian forever release the CYCA (and its directors, officers, employee, agents, and contractors) from all claims, demands, and causes of action relating to:

  • Compensation or remuneration for the CYCA’s collection, use, and disclosure of video, sound, or photographic recordings of the youth curler in the CYCA Promotional Activities; and
  • The collection, use, and disclosure of video, sound, or photographic recordings by third parties at CYCA events, whether such third party has performed the said collection, use, and disclosure on its own or whether such third party has collected, reproduced, or redistributed recordings obtained directly or indirectly from the CYCA Promotional Activities;

3. The CYCA’s use and disclosure of video, sound, or photographic recordings of the youth curler may involve identification of the name of the youth curler, the name of his/her curling team, and the position the youth curler plays on the team;

4. The youth curler and his/her parent or guardian may withdraw the consent they have hereby given to the CYCA’s collection, use, and disclosure of video, sound, and photographic recordings of the youth curler for use in the CYCA Promotional Activities. Such withdrawal of consent must be delivered to the CYCA in writing, and it shall be effective 10 days after the CYCA has received it.
